Commendation Request
This form is for National leadership to submit requests for commendations from the Knights of Peter Claver, Inc. Members / local leaders must ask the Deputy to complete requests if commendations are desired.
Submissions must be approved by the Supreme Knights / CEO before being produced / distributed.
Requests should be completed at least 3 weeks prior to date needed. This allows for timely creation, approval, and delivery.
Commendations for the following are accepted:
Requests submitted with shorter notice can not be guaranteed.
Submission does not guarantee approval.

- Significant Unit Anniversaries (10+year in min. of 5-year increments -25th, 30th, 35th etc.)
- Unit / Member Achievements with regional / national significance
- Significant Member Milestones (100 year birthday's; 75 year's of active/financial membership,etc)
- Affiliated Church / Organization Significant Milestones (Anniversaries / Religious Anniversaries / Retirements / Appointments)
