ALL ATTENDEES (YOUTH AND ADULTS) ARE REQUIRED TO COMPLETE THIS FORM IN ORDER TO ATTEND THE JUNIOR NATIONAL CONVENTION. 
Adults over the age of 18 must have a current Virtus Certificate. If you do not have a current certificate you may take the training and apply for one here
